Number 9 Salon abides by the standards listed below. We also affirm that we have expanded our sanitation protocols to fight the spread of COVID-19 and other communicable conditions.

Health Protocols for Customers

Our goal is to provide a safe environment for our customers and staff, and to advance the safety of our local community. This document provides information we ask you to acknowledge and understand regarding the COVID‐19 virus.

  • The COVID‐19 virus is a serious and highly contagious disease. The World Health Organization has classified it as a pandemic. You could contract COVID‐19 from a variety of sources. Our salon wants to ensure you are aware of the additional risks of contracting COVID‐19 associated with salon services.
  • The COVID‐19 virus has a long incubation period, during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not given the current limits in virus testing.
  • Consistent with the actions taken by many businesses across the state, and especially because of the close proximity between customers and staff while receiving salons services, wearing cloth face coverings (over the nose and mouth) is required by both customers and salon staff. If available, individuals should consider wearing non-medical grade face masks or multi-layer cloth masks at the very least. Wearing a mask is of utmost importance because of the close proximity between customers and salon staff while services are being provided inside the salon.
  • Maintaining at least six (6) feet separation from other individuals while inside the salon will also be necessary. Other measures such as: face covering, hand-washing hygiene, cough etiquette, workstation cleanliness and sanitation before and after interaction with each customer will be rigorously practiced by staff.
  • Even though we are limiting appointments, staggering the timing of visits by other customers, following mandated sanitization protocols and taking extra safety precautions, the characteristics of the virus and the close proximity required for our staff to provide salon services do present an elevated risk of you contracting the virus simply by being in our salon.

The virus that causes COVID-19 can be spread to others by infected persons who have few or no symptoms. Even if an infected person is only mildly ill, the people they spread it to may become seriously ill or even die, especially if that person is 65 or older with pre-existing health conditions that place them at higher risk.

  • Because of the hidden nature of this threat, everyone should rigorously follow the mandated practices and protocols specified by Executive Orders issued by State, County, and Local authorities, all of which facilitate a safe and measured reopening of Kansas. The virus that causes COVID-19 is still circulating in our communities. We MUST continue to observe practices that protect everyone, including those who are most vulnerable.

Fieldset

  • Release of Liability and Consent

  • I accept that there is an increased risk of contracting the COVID‐19 virus in the salon or by receiving salon services and I knowingly and willingly consent to have a salon/spa service during the COVID-19 pandemic. I further confirm and acknowledge that I could contract the COVID‐19 virus from outside this salon and due to circumstances unrelated to my visit here.

I understand the COVID-19 virus has a long incubation period, during which carriers of the virus may not show symptoms and still be highly contagious. It is impossible to determine who has it and who does not, given the current limits in virus testing.

To prevent the spread of contagious viruses and to help protect each other, I understand and will follow all State, County, and Local requirements as well as Number 9 Salon's strict customer guidelines.

I understand that air travel increases the risk of contracting and transmitting COVID-19 virus. I verify that I have not traveled outside of the United States in the past 14 days. I also verify that I have not traveled domestically within the United States by commercial airline within the past 14 days.

I affirm that I, as well as all household members have not been diagnosed with COVID-19 in the last 30 days.

I affirm that I or other household members have not knowingly been exposed to anyone diagnosed with COVID-19 within the past 14 days.

At the time of my appointment, if I I can answer "yes" to any of the self-screen questions above, I will cancel and reschedule my appointment at a time where 14 days has elapsed since the last symptoms and/or exposure have occurred.
